Client Intake Form – New Dog Before we begin, we’d like to get some details about your dog and your lifestyle. This will help us create a plan that fits your needs. The more of this basic information we cover beforehand, the quicker we can get to the good stuff (the training plan) in your onboarding session. A quick guide:-Your answers can be as brief or as detailed as you’d like.-Feel free to skip any questions you’d prefer not to answer.–If you have multiple dogs: You can answer the dog-related questions for the one whose behavior will be our main focus.
