Have they been to a vet since you brought them home?
Do you know anything about the dog's background/previous living situation? If so, please describe
If this is a puppy from a breeder: did you meet either of the parents? Do you know anything about them? For example, "purebred working-line dog," "mixed breed, accidental litter," "mom was shy," etc
Has this dog ever bitten a human or another dog, to the best of your knowledge? (Not including normal puppy mouthiness)

No
Maybe? My puppy has bitten, but I'm not sure if it's normal puppy mouthiness or not
If you answered "yes" to the above, please describe the bite incident(s)
Does this dog have any known health issues, like allergies, food sensitivities, pain, etc? Are they on any medications or supplements?
What do you feed them? (Dry food, wet food, homemade, etc)
What kind of treats do you use for training rewards, if any?
Does anyone else live in your home, whom you haven't already told us about? (Kids, roommates, etc). If there are kids, please include their ages.
Please list all other pets in the home, if any.
Please describe your home type. House? Apartment/flat? In the city? Suburbs?
Does your home have a fenced outdoor area where your dog can safely be off-leash?
Sometimes, we advise clients to take their dogs to parks or other away-from-home places to work on training or enrichment. To help us determine how feasible that is for you, please tell us about your "transportation situation." Do you have a car, or do you use public transport? If you have a car, how does your dog do on car rides?
How comfortable are you walking your dog - can you physically control them or are they too strong? Do you use any tools (no-pull harnesses, prong collars, head collars, etc) to make walks easier?
Please describe your current work or school situation. For example, do you work full-time, part-time, from home? Retired? Not working? Are you a stay-at-home parent? This helps us get a sense of your capacity for training and how much time your dog spends alone.
Is there anything else you'd like us to know before we start? (Don't worry about explaining details of your dog's behavior challenges yet. We'll discuss that at your first coaching session)
