Do you know anything about the dog's background/previous living situation? If so, please describe
Is this dog de-sexed (spayed or neutered)?
Yes
No, and we probably won't get that done
No, but we intend to get it done at some point
Does this dog have any known health issues, like allergies, food sensitivities, pain, etc? Are they on any medications or supplements?
What do you feed them? (Dry food, wet food, homemade, etc)
Has this dog ever bitten a human or another dog, to the best of your knowledge?
If you answered yes to the above, please describe the bite incident(s)
Has this dog had any previous training? If so, please tell us a bit about it. For example, what kind of things did you work on? Was it a group class, board-and-train, private lessons, training on your own, etc?
Please list all members of the household. If there are children, how old are they?
Please list all other pets in the home, if any.
Please describe your home type. House? Apartment/flat? In the city? Suburbs?
Does your home have a fenced outdoor area where your dog can safely be off-leash?
Sometimes, we advise clients to take their dogs to parks or other away-from-home places to work on training or enrichment. To help us determine how feasible that is for you, please tell us about your "transportation situation." Do you have a car, or do you use public transport? If you have a car, how does your dog do on car rides?
How comfortable are you walking your dog - can you physically control them or are they too strong? Do you use any tools (no-pull harnesses, prong collars, head collars, etc) to make walks easier?
Please describe your current work or school situation. For example, do you work full-time, part-time, from home, are you retired, or a stay-at-home parent? This helps us get a sense of your availability for training and how much time your dog spends alone.
Is there anything else you'd like us to know before we start? (Don't worry about explaining details of your dog's behavior challenges yet. We'll discuss that at your first coaching session)
